New advances in minimally invasive fusion surgery techniques can relieve back pain without the trauma of traditional spinal surgery and patients can be back on their feet in a matter of weeks, not months. One of the most promising new types of minimally invasive back surgery is XLIF, or eXtreme-Lateral Interbody Fusion, says Kaixuan Liu MD, PhD, chief surgeon at Atlantic Spinal Care in Edison, N.J., an expert in the cutting-edge technique. Dr. Liu says. “The benefit to the patient with XLIF is a less painful surgery with fewer traumas to the body than in traditional open fusion surgery because it avoids traversing through the abdomen or cutting and disrupting the muscles in the back,” adds Dr. Liu.

Minimally Invasive Fusion

Spinal fusion surgery has been around since the early 1900’s. It was originally developed to correct spinal deformities caused by tuberculosis, and has since become a widely accepted surgical treatment for many kinds of back pain. But the new XLIF procedure is far superior to older techniques, for several reasons, Dr. Liu explains.

XLIF is minimally invasive using very small incisions, typically less than half an inch long, instead of the single large incision that’s required in traditional “open” spinal surgery.
It also uses x-rays, which allow the surgeon to see without spreading apart the muscles and other tissues surrounding the spine.
The incisions are fitted with small dilator tubes that allow the surgeon to remove the damaged disc (or other material) and perform the fusion without creating unnecessary trauma to the surrounding area.
XLIF also uses an advanced nerve monitoring system that provides accurate, reproducible, real-time feedback about nerve health, location, and function, reducing the chances of nerve injury.

“The differences between XLIF and traditional spinal fusion for patients are enormous,” Dr. Liu says. Instead of a three- to five-day hospital stay, many patients can go home right away. They’re also typically up and walking the same day—vs. being immobile for two to four days with traditional fusion. “With XLIF, you can resume your normal activities in four to six weeks, in most cases,” he says. “That’s a big advantage over the six months-plus that you’d have to wait with a traditional operation.”

XLIF procedures can be used to treat many conditions, including degenerative disc disease in the lumbar spine, low-grade spondylolisthesis (a condition in which one of the vertebrae in the back slips out of position), degenerative scoliosis (curvature of the spine), and recurrent lumbar disc herniation (a condition in which the gel-like center of one or more of the discs in the spine has bulged or broken through the disc’s tough outer membrane). In most XLIF surgery, the surgeon will remove an intervertebral disc and fuse the space with a modern implant, which helps stabilize and maintain the spine’s natural shape and help it heal.

The technique is called “extreme-lateral interbody fusion” because the surgeon operates on the spine through a small incision on the side (the extreme-lateral side of the body), not on the abdomen (anterior side) or back (posterior side). It’s an “interbody” procedure because it addresses the area between the bones (vertebrae or discs) in the spine.

A new Canadian study of the mechanisms of aluminum adjuvant toxicity in pediatric patients confirms that immune challenges during early development, including those vaccine-induced, can lead to permanent detrimental alterations of the brain and immune system function. Summary

Immune challenges during early development, including those vaccine-induced, can lead to permanent detrimental alterations of the brain and immune system function. Experimental evidence also shows that simultaneous administration of as little as two to three immune adjuvants can overcome genetic resistance to autoimmunity. In some developed countries, by the time children are 4 to 6 years old, they will have received a total of 126 antigenic compounds along with high amounts of aluminum (Al) adjuvants through routine vaccinations. According to the US Food and Drug Administration, safety assessments for vaccines have often not included appropriate toxicity studies because vaccines have not been viewed as inherently toxic. Taken together, these observations raise plausible concerns about the overall safety of current childhood vaccination programs.

When assessing adjuvant toxicity in children, several key points ought to be considered:

1)    During prenatal and early postnatal development the brain is extremely vulnerable to neurotoxic insults;

2)    Aluminum is a neurotoxin and a strong immune stimulant. Hence, aluminum has all the necessary biochemical properties to induce neuro-immune diseases. Autism is one such disease. Namely, autism is characterized by dysfunctional immunity and abnormalities in brain function;

3)    In adult humans aluminum vaccine adjuvants have been linked to a variety of serious autoimmune and inflammatory conditions, yet children are regularly exposed to much higher amounts of aluminium from vaccines than adults;

4)    It is often incorrectly assumed that peripheral immune challenges (analogous to vaccinations) do not affect brain function. However, it is now clearly established that there is a cross-talk between the nervous and the immune system. It is also demonstrated that this cross-talk plays a crucial role in both immunoregulation as well as brain function. In turn, perturbations of the neuro-immune regulatory system have been demonstrated in many autoimmune diseases and are thought to be driven by a hyperactive immune response;

5)    The same components of the neuro-immune regulatory system that have key roles in both brain development and immune function are heavily affected by aluminum adjuvants;

In summary, research evidence shows that increasing concerns about current vaccination practices may indeed be warranted. Because children may be most at risk of vaccine-induced complications, a rigorous evaluation of the vaccine-related adverse health impacts in the pediatric population is urgently needed.
